The information found on the PSR is taken from several different information systems. To properly update the PSR, you need to update the correct information database. The following sections are the most likely to require updating:

A. Missing fitness reports: If a fitness report is not on your PSR and not in your microfiche, you need to first call PERS 311 at 901-874-3313 to see if they have any record of the fitness report being submitted. It is possible the fitness report was submitted, but returned to the reporting senior for a correction. Once you have them on the phone, they will tell you what action to take. If fitness reports are in your microfiche, but not on your PSR, call PERS 311 and request they pull your microfiche and update your PSR. We routinely notify PERS 311 of these discrepancies when we identify them while doing a review or an audit. If a fitness report is on your PSR, but not in your microfiche, you should send a copy to PERS 311 pointing this out and requesting that they have it filed in your microfiche.

B. Education: Education is provided by the NPC (PERS-912). They take the information from your Naval Reserve Qualification Questionnaire (NRQQ). Attach a copy of the new or missing degree to your annual NRQQ submission and it will be entered as appropriate into the system. NPC forwards a copy of the transcript/degree to PERS 312C for filing in your microfiche record. If the information is on the PSR, but the transcript is not in the record, forward a copy of the transcript or degree directly to PERS 313C for filing. Ensure your social security number is on the document. You can call PERS-912 at 1-866-827-5672 for additional assistance as needed. The mailing address is

Navy Personnel Command
PERS-912
5722 Integrity Drive
Millington, TN 38054

C. Personal Awards: If you lack personal award information on the top sheet of your OSR/PSR or not all your award citations are present in your official service record, new procedures are now in place to help you correct the situation almost effortlessly. Under a new procedure adopted by CNRFC, the frustration factor in trying to update records is virtually ZERO. All you need to do is forward to CNRFC the documentation; they will do the rest. CNRFC will enter the award information into the Navy-wide awards database and then forward the hard copy certificates and/or citations to Navy Personnel Command (PERS-312) for digital filing in your official service record. It couldn't be easier. When sending award documentation to CNRFC, be sure to place your social security number, clearly written, on the upper left corner of the document.

For database entries, just go to the NDAWS site and click on “Personal Awards,” sign on, and review earned awards in the database under your name. For official service record entries, wait a sufficient amount of time to allow digital processing; then, order a CD copy of your record.

D. Service Schools Attended: This information is not included on Reserve PSRs.

E. Present duty station (unit) is taken from NSIPS data base

F. Present billet is not included on PSR.

G. Special Qualifications are extracted from the database. If you have
questions on approved NOBCs not appearing in this section, you should call PERS- 912 customer service desk at 1-866-827-5672.
